We are owned by InHouse Care LLC which provides Advanced Practice Registered Nurses to Nursing homes and Assisted Living facilities throughout the state of Connecticut.

We can assign practitioners to your facility to evaluate and treat your residents. We also have the ability to contract with your home to send APRNs to the facility if you are in need of support.

InHouse Care has relationships throughout the state with multiple specialist groups. We purposely do not employ specialists in our group so that your facility can maintain its referral sources.

We will assist with the referral process and many specialist practices offer telehealth visits to their patients.

In Person Support

Sometimes telehealth just isn't enough. We can contract with your facility to provide a regular in person APRN to see patients.

Blended Model

If the patient is unable to do the telehealth visit alone or with the support of a facility nurse, we may be able to provide a staff member to come to facility to assist with visit.

Telehealth Support

Use of multiple forms of technology to make telehealth visits easy for the patient. Can be done without the support of facility staff or with nursing support.

Remote Patient Monitoring

We will send medical equipment to your patients that will allow us to remotely monitor blood pressure, pulse, blood sugars, and more if needed.
